Advance Directive Forms are available online from the Nevada Center for Ethics and Health Policy (NCEHP) at www.unr.edu/ncehp/ads.html.

A step-by-step advance directive process is also available through NCEHP at www.nvlivingwill.com.

The Nevada Secretary of State has developed a new program called the "Living Will Lockbox".
It is an online program that allows users to register living wills, advance directives and DNR information
and then give certain people (e.g. health-care providers) permission to access the info when they need it.  
The program, along with more information, is provided at http://nvsos.gov/online/nadr/.
